Personal Medical Record Software

This computer program, tentatively called 'Gracie', enables individuals to maintain their own medical records on a flashdrive (or memory stick). Written in Perl, it provides for a complete database and problem-oriented record of visits with Providers

The program is based on a concept originated by Dr. Lawrence L. Weed, and published in the books 'Problem-Oriented Medical Record' and 'How to Manage Your Health Care'.

Gracie is organized around two principal concepts:

1. A Complete and Accessible Medical Database;
2. A Problem-Centered Record of Diagnosis and Treatment.

The Data Base consists of nine data files plus a Review of Systems file. The nine data files are:

1. Personal Information
2. Family History
3. Personal Medical History
4. Immunizations
5. Allergies
6. Medications
7. Use of Substances
8. Emotional and Social History
9. Operations and Emergency Procedures

The Review of Systems file contains data (notably symptoms and signs) collected during a self-review or Provider review of the following systems: Constitutional, Eyes, Ears, Nose, Mouth, Throat, Head, Skin, Cardiovascular, Respiratory, Neurological, Hemotologic, Urinary, Genital, Musculoskeletal, Gastrointestinal, Thyroid, and Psychiatric.

A 'Problem' is a physical, mental, or social condition which has been diagnosed (i.e., identified and named) as a disease or abnormality, the underlying cause of a symptom or sign, or set of symptoms or signs, requiring treatment and/or management. Problems are identified by patients and Providers. Problem records include Progress Notes, Plans for Treatment, Diagnoses, Results of Examinations, Self-Treatment by patient, and summaries of discussions between Providers and patients.
